The report should be in writing and should contain: The name of the student, student group, or student organization suspected of the violation(s). All details of the suspected violation(s), including the date, which are known to the person reporting it. The names of potential witnesses to the suspected violation.

Any person not a member of the University community may also report suspected violations of the codes and should follow these same procedures.

To the extent feasible, those reporting are asked to use the Reporting Form located on the Spalding Portal or go directly to the form at http://edu.4civility.com/frontpage/spalding.cfm
